We make it simple to connect with licensed wound care providers who deliver specialized treatment directly to your home under your physician's orders.

    01

    Contact Us

Call or submit a short form to get your healing journey started. Our intake team is available to answer your initial questions and gather the basic details about your wound care needs. Whether you reach out by phone or through our secure online form, we make the first step as simple and stress-free as possible.

    02

    Information Review

We review your needs, insurance, and physician's orders. To ensure a smooth process, we verify your insurance coverage and confirm we have the necessary medical orders from your referring physician. This careful review helps us understand the specific clinical requirements of your wound care plan.

    03

    Provider Matching

We match you with a licensed provider based on your condition. Using the details gathered, we connect you with an independent, vetted clinician from our network who serves your specific area in Broward County. We ensure the matched provider has the right expertise for your particular wound type.

    04

    In-Home Visit

The matched provider reaches out directly to begin intake. Once matched, the licensed clinician will contact you to schedule your first in-home visit at a convenient time. During this initial visit, they will assess your wound, review your physician's orders, and begin the prescribed care plan.

    05

    Ongoing Care & Monitoring

The provider delivers treatment and manages follow-up as needed. Your clinician will continue to visit your home on the schedule prescribed by your physician, providing consistent dressing changes and monitoring. They will keep your referring doctor updated on your healing progress every step of the way.

    Frequently Asked Questions

Common questions about getting started with our provider network.

How long does provider matching take?

Most patients are matched with a licensed provider within 24 to 48 hours of providing their physician's order and insurance information.

What information do I need before I call?

Have your physician's order for wound care, your insurance card, and a brief description of your wound type or condition. That's all we need to begin the matching process.

Do I need a doctor's referral to get in-home wound care?

Yes. A physician's order is required before care can begin. Your primary care doctor, surgeon, or specialist can provide this. If you're unsure what to request, call us and we'll guide you.

What types of insurance do the providers in your network accept?

We work with providers who accept Original Part B Medicare, Preferred Care Partners, United Dual Choice D - SNP Plan, and WellCare. Call us to verify your specific plan before your first visit.

What should I expect during the first in-home visit?

The visiting clinician will review your physician's orders and medical history, assess your wound, perform an initial dressing change if appropriate, and walk you through the ongoing care plan.
